Apply for a Grant - As a Patient
In terms of the individual patient, typically we are receptive to grant requests for assistance with the little things to help ease the burden of every day treatment.
It is important to note that we do not give cash grants to individuals, nor do we cover the cost of medical treatments.
What we can do is assist with basic needs that ease the financial stress patients and their families face by paying bills (electric, phone, water, gas, etc.), rent, tutoring, small toys, transportation and other incidental needs that arise.
Application Procedures:
Due to the overwhelming demand for assistance that our organization continues to experience, and the limited resources at our disposal, we have made significant changes to our application procedure. These new measures are outlined below.

IMPORTANT INFORMATION
This application is your request to Team Continuum to consider paying essential bills while in treatment. Unfortunately, submitting an application does not automatically mean we agree to pay any of the bills you submit. Please continue paying them until or unless you receive a call from our office telling you that Team Continuum will make the payment(s). We are experiencing an increased volume of requests. Please understand our grants are based on a needs assessment, limited staff, and availability of funds.
PROCEDURE
Your application must be in complete compliance before it can be accepted for consideration.
- Applications will be accepted between the 1st and 15th day of the month.
- Applications received after the 15th day of the month, whether by mail, fax or email, will be either discarded or returned.
- Section 6: Must be completed by Social Worker (not an "intern" or volunteer). Do not submit your application without completing this section. It cannot be left blank.
- Section 7: Must be completed by your Oncology MD or Oncology RN.
- Only applications containing ALL information, ALL bills, and ALL other required documentation will be considered.
- Applications with incomplete sections, and/or missing bills and documents will be returned.
- It may be a month or more before we can review your completed application.
