Apply for a Grant - As a Patient

In terms of the individual patient, typically we are receptive to grant requests for assistance with the little things to help ease the burden of every day treatment.

It is important to note that we do not give cash grants to individuals, nor do we cover the cost of medical treatments.

What we can do is assist with basic needs that ease the financial stress patients and their families face by paying bills (electric, phone, water, gas, etc.), rent, tutoring, small toys, transportation and other incidental needs that arise.

Application Procedures:

Please include an IRS W-9 Form. This is very important for our accounting purposes, however you will not be taxed. Please be aware that in cases where rent is paid, we will need your landlord to fill out a W-9 as well.

Please include invoice/bill copies of all requests when applicable. This will help expedite the payment process.

Helpful Tips Before You Apply:

If you are seeking mortgage assistance, please make every effort to lower your rate with your bank, and contact your congressman.

If you need help with a financial loan, you should be able to receive a forbearance on your loan due to medical hardship through NY State and HESC.

Read some of the letters of thanks and you will get an idea of some of the assistance we have provided.

We also pride ourselves on being able to make quick and compassionate decisions. We understand that if a patient needs something, they usually need it quickly, and so while there is an application process, it is a relatively straightforward and we are always here to help you.

We give every application serious consideration, and do our very best to respond quickly. It is always helpful to give us a good overall picture of the patient or family circumstances, and prioritize the needs.

Each request for assistance goes to one of our approval committees for consideration, and each decision is based on the merits of the circumstances. Please remember, we only offer assistance for Non-Medical needs and do not give cash awards to any individuals, in every case, without exception, we only consider support where Team Continuum can make payments to vendors or suppliers on behalf of the patients, and that can be substantiated with a bona fide receipt.
